What direct-pay care means
Direct-pay care means paying the practice directly for the services you receive. It is not health insurance. Review the professional fee and what it includes before starting care. Prescription medications, laboratory testing, supplies and outside services are separate unless your written plan specifically includes them. You can request a Good Faith Estimate.
Services for Arizona adults
Our services include medical weight management, men's hormone health and TRT evaluation, women's hormone health and menopause care, erectile dysfunction and premature ejaculation care, medical skincare, adult ADHD / ADD evaluation, general medical care and combined metabolic and hormone care. Telehealth is available for adults located in Arizona. Select in-person visits in the Northwest Valley are available by arrangement; the practice will confirm the appropriate visit format.
Start with your service intake
Choose your service on our Patient Forms page and complete the matching intake. Some weight management and sexual health requests can begin with an online assessment. Your clinician reviews the information and determines whether a visit, tests or additional information are needed before making treatment recommendations. Completing an assessment does not guarantee a prescription.
Adult ADHD / ADD evaluation and TRT require both a completed service intake and a scheduled appointment. After completing the relevant form, request an appointment. An appointment request is not a confirmed appointment; the practice will confirm the details.
Know the fees before you begin
Our Programs & Pricing page lists the current professional fees for initial evaluations, follow-up care and ongoing programs. It explains which services are included and which costs are separate. ADHD medication costs are separate from visit fees. Medication selection and pharmacy costs depend on the individual treatment plan, so ask the practice about your expected total cost before proceeding.
