Terms: Limited to the first 50 new patients across Hamel Direct Care. One discounted initial consultation per new patient. Follow-up visits, ongoing management, medications, labs, supplies, shipping, injection or infusion administration, and outside testing are excluded from the discount. The practice confirms offer availability and your fee before care. Treatment and prescriptions depend on clinical appropriateness.

Clinical note: ADHD and TRT require both completed intake forms and a scheduled appointment. For other services, the clinician confirms next steps after reviewing your intake.

Existing patients: Follow the fees in your current written care agreement. Contact the practice with any questions about your agreement.

Amounts listed for semaglutide–glycine and tirzepatide–glycine show the semaglutide or tirzepatide content per vial, excluding glycine. These are vial prices, not monthly supply prices. Vial selection and usable supply depend on your prescription and applicable discard dates.

The 50% introductory offer applies only to the initial consultation professional fee for the first 50 new patients across Hamel Direct Care, once per patient. Medication and follow-up or ongoing management fees are not discounted.

Medication is prescribed only when clinically appropriate after evaluation. A consultation does not guarantee a prescription. Labs, dispensing charges, shipping, supplies, and injection or infusion administration services are additional when applicable; additional charges will be confirmed before payment.

Compounded medications are not FDA-approved. They are considered only when an FDA-approved medication cannot meet an individual patient's medical needs. FDA information about compounded GLP-1 medications

Signature Program

Combined Metabolic & Hormone Health Evaluation — $249

An extended evaluation combining medical weight management with men's or women's hormone health. Includes combined medical assessment, laboratory planning and one results-review appointment. Laboratory, medication, supplies and shipping costs are separate. TRT requires its own intake form and a scheduled appointment.

Ongoing Combined Management — $169 per month

Includes one coordinated follow-up assessment per month, related laboratory review, routine current-plan questions and treatment coordination. This replaces separate weight and hormone monthly fees for the services included in the combined plan. Laboratory, medication, supplies and shipping costs are separate. Enrollment is offered only after the initial evaluation and when ongoing treatment is clinically appropriate.

This program combines metabolic and hormone care. If your plan includes TRT, both a completed TRT intake and a scheduled clinical evaluation are also required.

Medical Weight Management

Medical Weight-Loss Evaluation

Includes individualized assessment, medication-risk review, treatment planning and appropriate laboratory orders. Laboratory charges are separate. A personalized medication quote follows clinical review; medication, dose, pharmacy, supplies and shipping affect total cost.

Ongoing Weight Management

$89 per month

After the initial evaluation. Includes one follow-up assessment each month (online review or appointment as clinically needed), related laboratory review, treatment and refill decisions, and routine questions about the current plan. Medication, laboratory and supply costs are separate.

Weight-Maintenance Visit

For established stable patients when clinically appropriate. Frequency is individualized. Not charged concurrently with an ongoing monthly plan for the same included service.

Men's Hormone Health

Men's Health and Low-Testosterone Diagnostic Package

Hamel Direct Care is a direct-pay medical practice and does not submit insurance claims for these programs. Professional fees cover only the clinical services specifically listed. Prescription medications, laboratory testing, imaging, medical supplies, pharmacy dispensing, shipping and outside specialist services are separate unless expressly stated. Completing the intake form begins clinical review; the practice confirms the appropriate service and total expected cost before payment or treatment. Payment does not guarantee a prescription, a particular medication, a specific treatment or a particular outcome. Itemized receipts or superbills are available upon request; reimbursement is determined by the patient's health plan and is not guaranteed. HSA or FSA cards may be accepted for eligible medical expenses, subject to the patient's plan requirements. General wellness and cosmetic expenses may not qualify. Medicare and AHCCCS members should contact Hamel Direct Care before booking so eligibility for direct-pay services can be confirmed.

Your Right to a Good Faith Estimate

Patients who are uninsured or who choose not to use insurance may request a written Good Faith Estimate of expected charges. If the final bill from a provider is at least $400 more than that provider's estimate, the patient may be eligible to dispute the bill. Contact Hamel Direct Care before receiving services to request an estimate.

Pricing questions

Do you bill insurance for these programs?

Hamel Direct Care is a direct-pay practice and does not submit claims for these programs. Itemized receipts or superbills may be requested, but reimbursement is not guaranteed.

Are medication and laboratory costs included?

No, unless a service specifically states otherwise. Medication, laboratory, pharmacy, supply and shipping charges are generally separate. For weight management medications, a personalized quote follows clinical review — medication, dose, pharmacy, supplies and shipping all affect total cost.

Does paying for a consultation guarantee medication?

No. Payment covers the medical evaluation. Prescriptions are provided only when clinically appropriate. Extra services are discussed and quoted before proceeding.

Can I use an HSA or FSA?

HSA or FSA cards may be accepted for eligible medical expenses. Eligibility depends on the medical purpose, applicable rules and the patient's individual plan.

I am an existing patient. Which fees apply to me?

Existing patients should follow the fees in their current written care agreement. Contact the practice with any questions about your agreement.
